DO WE BILL INSURANCE?

Insurance can be confusing, so we try to make it as simple as possible while also protecting our small business.
At Luna Wellness, we work with a few select insurance networks, and coverage can vary depending on your provider, plan, and the practitioner you see. Some of our practitioners are in-network with certain plans, while others are considered out-of-network.
We are happy to check benefits for you after you have an appointment on the books. Our intake forms will prompt you to submit your insurance cards and information. We need this information at least 48 hours before your appointment: (If you appointment is Monday we need this information Friday)
If we do not receive this info we will do our best to contact you and check in, but if we are unsuccessful and are not able to get in contact, we will cancel your appointment. If you cannot get to the intake forms, at the very least email us with the following information:
A photo of the front and back of your insurance card
Your full name and date of birth
Relationship with the policy holder if it is not yourself
If we’re unable to verify your benefits before your appointment, you’ll be responsible for the full session amount at the time of service. Once verification is complete, any eligible reimbursement will be applied or refunded accordingly.

Patient Responsibility:
Please remember your insurance plan is a contract between you and your insurance company, not between you and our clinic. Payment is dependent on eligibility of services at time of service and all terms and conditions of your plan. You are responsible for any charges not covered by insurance. For specific questions regarding your unique health plan and benefit, please call your insurance company directly.
A debit/credit card is required to hold your appointment in the event of a late cancellations/no show which do result in a fee.

Can I get a massage using insurance?
If you are looking for a massage to book using health insurance, we must bill under Medical Massage. Therapists that are available to book will allow you to book the appointment under “Medical Massage” Please confirm on the chart on this page as to which providers are in with what network.
To avoid delays with getting reimbursed we require a referral for a massage unless you are seeing Dr. Fisher here at the clinic. This includes MVA/PIP claims.
Can I bring my newborn in under insurance?
Babies coming in to see our Chiropractor will need their own insurance card. We are not able to accept insurance when new little one is still on mama’s insurance. We unfortunately run into problems that can cause major delays with payment. We are able to supply a superbill for these visits and once your little one has their own insurance cards we are happy to bill on your behalf.
What if you don’t take my insurance, but I still want to get reimbursed?
If we do not take your insurance or you wish to see a provider not in network but still within the companies the clinic is in network with, we are able to provide you with a superbill, which is an itemized receipt with diagnosis and billing codes that can be submitted to your insurance company for reimbursement.
Please let us know when checking in that you need a superbill.
Motor Vehicle Accidents
We DO bill insurance for car accident claims. (PIP/MVA CLAIMS) Please call the office before making your appointment and have the following information ready: Date of incident, claim number, adjuster’s name, and number. If we do not have this 24 hours before your appointment we may cancel until this information is done. WE must verify your claim BEFORE we see you.
If you are only coming in for a massage, we require a referral from your doctor.

If I purchase a package or membership, can I still get a Superbill for my services?
Yes, after your appointment (or month if you have the unlimited acupuncture). We will provide a superbill for you to submit. this is great if you still need to meet your deductible, or we are not in network.
