Insurance can be confusing, and coverage varies widely between plans. To help avoid surprises, we encourage all patients to contact their insurance provider directly to understand their benefits before their appointment.
Do We Bill Insurance?
Luna Wellness is happy to submit claims to your insurance company when we are in-network with your plan. However, we do not relay individual insurance benefits to patients prior to their appointment.
Insurance coverage varies widely between plans, and the details of your benefits are part of the contract between you and your insurance provider. For this reason, it is the patient’s responsibility to contact their insurance company directly to understand their coverage, including:
• Deductibles
• Copays or coinsurance
• Coverage for chiropractic, acupuncture, or massage therapy
• Referral or authorization requirements
While we are happy to submit claims as a courtesy, insurance verification by a clinic does not guarantee payment, and patients are ultimately responsible for any portion of services not covered by insurance.
Insurance Information Required Before Your Appointment
Please provide your insurance information at least 48 business hours before your appointment.
If insurance information is not provided ahead of time and you bring your card to your appointment, the full appointment cost will be due at the time of service. Once insurance processes the claim and payment is received (which may take 6–10 weeks), any applicable reimbursement will be issued.
This policy applies to your first visit only.
To submit your insurance, please send:
• A photo of the front and back of your insurance card
• Your full name and date of birth
• Your relationship to the policy holder (if different)

Appointment Hold Policy
A valid debit or credit card is required to hold your appointment.
Late cancellations and no-shows may result in a fee of up to $100.

Massage & Insurance
Can I Book Just a Massage Using Insurance?
Massage therapy billed to insurance must be scheduled as Medical Massage.
When booking online, please select “Medical Massage.” Providers who offer insurance-based massage will appear under this service option.
If a massage is booked under a non-medical massage service, the full appointment cost will be due at the time of the visit.
Please review the provider chart on this page to confirm which providers are in-network with your insurance plan.
Referral Requirement for Medical Massage
To avoid delays with insurance reimbursement, we may require a referral for medical massage unless you are currently seeing Dr. Fisher at Luna Wellness.
This referral requirement also applies to MVA/PIP claims.
Pediatric Chiropractic Insurance
Can I Bring My Newborn Using Insurance?
Babies receiving chiropractic care must have their own insurance card.
Unfortunately, we cannot bill insurance when a newborn is still listed under the mother’s insurance without their own policy identification. This situation frequently causes claim delays and payment issues.
We are happy to provide a superbill for these visits so you may submit it to your insurance company for possible reimbursement.
Once your child receives their own insurance card, we will gladly bill insurance for future visits.
Out-of-Network Insurance
What If You Don’t Take My Insurance?
If we are not in-network with your insurance, you may still be eligible for reimbursement.
We can provide a superbill, which is an itemized receipt containing the diagnosis and billing codes needed for you to submit a claim directly to your insurance company.
Please let our front desk know at check-in if you would like a superbill.
Motor Vehicle Accidents
We DO bill insurance for car accident claims. (PIP/MVA CLAIMS) Please call the office before making your appointment and have the following information ready: Date of incident, claim number, adjuster’s name, and number. If we do not have this 24 hours before your appointment we may cancel until this information is done. WE must verify your claim BEFORE we see you.
If you are only coming in for a massage, we require a referral from your doctor.

If I purchase a package or membership, can I still get a Superbill for my services?
Yes, after your appointment (or month if you have the unlimited acupuncture). We will provide a superbill for you to submit. this is great if you still need to meet your deductible, or we are not in network.

Insurance companies often process claims weeks after your visit, and the amount they pay depends on your specific plan. If your deductible has not been met or your plan includes coinsurance, you may still owe a portion of the visit after insurance processes the claim.
Because of this, balances may appear after insurance has finalized the claim.

A deductible is the amount you must pay out-of-pocket each year before your insurance begins contributing to covered services.
For example, if your deductible is $1,500 and you have not yet met it, insurance may apply the entire visit cost toward your deductible rather than paying the claim.

Coinsurance is the percentage of the visit cost that you share with your insurance company after your deductible has been met.
For example, if your plan covers 80% of services, you may be responsible for the remaining 20%.

Massage therapy is only eligible to be billed to insurance when it is scheduled as Medical Massage and meets the requirements of your specific insurance plan.
Some plans require a referral or diagnosis, and many plans do not cover massage therapy at all.
We recommend contacting your insurance company to confirm your benefits before scheduling.

If your insurance does not cover the service or we are not in-network with your plan, you are still welcome to receive care at Luna Wellness.
We can provide a superbill, which you may submit to your insurance company for possible reimbursement depending on your plan.

• Insurance processing errorsIf a claim is denied, the balance becomes the patient’s responsibility.

Insurance processing times vary but typically take 4–10 weeks.
Once the claim has been processed, any remaining balance will appear on your account and we will notify you.
